NebuCare Program

1.What is NebuCare?

• SuperCare establishes an inventory of Nebulizers in MD’s offices and manages the levels at SuperCare
• A Nebulizer program designed for Doctor’s offices and clinics who prescribe Nebulizers to their patients
• Provides patient and physician convenience for automatic dispensing at MD’s offices
• Better patient care with immediate treatment with little paperwork

2.Which patients are eligible?

• Patients with chronic diagnosis
• Straight Medi-Cal patients, Blue Cross Fee for Service and Medicare

3. What type of paperwork is involved?

• Simple Intake Form or On Line capability
• The patient or guardian is required to sign all key documents
• Nebulizer Serial # is required on sale ticket
• A prescription, patient demographics and copy of insurance or printout is required for each Nebulizer dispensed
• Pre-stamped Pre-postage envelopes are made available to closets for mailing of all paperwork for Nebulizers dispensed
